I thought she was still filming the Anne Rice movie when she died, but I'm not a hundred percent sure.
When I saw this thread title Brandon Lee, John Candy and Heath Ledger were the first that came to mind. And WOW, that Vic Morrow footage was crazy. I remember hearing about that when I was a kid but never saw that clip.

Yeah, the video is posted on page one. It's pretty brutal, but it basically just shows the helo crashing in to them, you won't see any blood or guts. It's just one of those things where you know they didn't survive. Kind of like watching a ton of bricks falling onto someone, you don't have to see the person to know they're dead. Unless of course that person is Jason or Freddy.

One of the most famous was Marilyn Monroe dying during the filming of Something's Got to Give co-staring Dean Martin. A few years later it was re-made as Move Over Darling with Doris Day and James Garner.
Something's Got to Give with MM was eventually finished and released. Interestingly Wally Cox played in both versions.

The woman that played the Oracle in the Matrix and the guy that played Dumbledore in Harry Potter both died in between movies in their respective series. Both had to come up with cheesy dialogue to explain why their character looked different
