If I had to guess I'd say this is what happened in this situation. Facing backwards, hit a bump and the back of his head whiplashed into the ground.

Seems to vary by location. Rules for golf carts here (no mention of seatbelts):

They said he fell off and hit head first. I never knew whether it was front or back. I do know he sat up, and then threw up. Then he immediately lost consciousness.

I fell off a golf cart when I was maybe 12 years old or so, brother's GF was flying and went too fast around a corner and I went flying out. Luckily I landed on my elbows and skinned them clean but that was it.

A low speed vehicle is just a golf cart that has been made street legal. Street legal is lights, seat belts, governor (speed limited to between 20-25mph (has to go over 20 but under 25)), mirrors, etc.
Some communities allow non street legal carts on the roads. But more and more require them to be street legal which requires registration and therefore insurance.

Yup, I'm aware of all of that. Just saying that you can drive a golf cart on the streets here, legally, and it doesn't have to have a seatbelt, as was previously suggested.

Gated neighborhood by chance? If so, the "street legal" rules don't apply since its private roads. HOA basically sets the rules in a gated subdivision.
But if you're riding on public roads, you need to have seatbelts, rear view mirrors, turn signals, brake lights, headlights, and a speedometer to be street legal. And it needs to be registered and have liability insurance just like a regular car. They're considered "low speed vehicles" to be street legal. At least that's how it is in Louisiana and Florida.
ETA: maybe I'm partially wrong. LA requires all of the other equipment I listed, but I don't see seat belt on the LA last. Florida's requirements do list seatbelts for every seat though.
