Tkachuk shouldn't have even been out of the box. He was serving a matching minor roughing with Kane and there had been no stoppages. The D who committed the hold on McDavid should have been out. They let the wrong player out, unless NHL is different.

There's the explanation. That must be an NHL rule. USA Hockey rules are different. Same with that offside call. If the puck is under possession and passed back, yes, that is not offside. But that was a deflected puck, not controlled. Pretty sure that is offside in USA Hockey rulebook.
