The only thing you listed that was a legitimate reason to not send him was the score/inning
If the bases are going to be loaded for a pitcher struggling to find the strike zone and either our best hitter or best OBP guy is up. Then I take my chances not sending him.
I will say that MOST of the time you should send him but there are no ALL THE TIME rules for coaching 3rd base. That's all I'm trying to say. I dont think PM thinks there are any either.

He did coach 3rd tho. A lot of people don’t remember that because most of the best fans in college baseball didn’t follow the team that season.
Many of us season ticket holders sat in HR Village drunk as frick and came in for an inning or two.

3rd base coach is a no win job - damned if you do, damned if you don't. Sometimes you take chances depending on how game is playing out. Sometimes you take chances if you've done pregame scouting and you know OF's arm strength.
Most of the time it takes a perfect throw and you are guessing it won't be a perfect throw and it's a gamble. If the other team makes a great play, that's part of the game. You also have the judgment of an umpire and video review that could come into play as well.
I do think Cain was overwhelmed when he had to do it that first year he was an assistant. He seemed to run LSU out of a lot of innings. He has gotten better and really at the end of the day it's on PM.
