This is for a screened porch so I don't need anything super strong or perfect. I just need concrete to place some outdoor furniture on to drink my coffee from and the occasional entertaining a few times a year.
What kind of cost can I expect?

Id say about $1500. Can probably find a concrete worker to do it on the side.
Thats what I did. He formed it and poured it on a Friday afternoon. Came back Saturday to remove forms. Did all the work by himself.
I was going to just do it, but I'm glad I didnt.

You have 3 cy of concrete at a 4” depth. Concrete suppliers tropically will not sell you 3 cy at $105/ as it is considered a short load. They have trucking, driving, insurance, etc costs figured into their $/cy.
Figure $800 for concrete.
What is the current state of the area the new slab is to be placed? Assuming it needs to be grubbed and de-grassed, you need to figure haul-off or left for you to deal with.
A small concrete guy will do this work for $1,000 or so to form, place, and spread. You will need to provide the lumber for them to form 9 times out of 10. $250 for form material.
Now to the placement method... Can the supplier tailgate the concrete right out of the truck, or will you need equipment to place? A Georgia buggy runs you around $250 for a day’s rent.
I’d say $2,500 is a good price for this work all-in.

Why not just call around and get quotes straight from the horse's mouth.

Probably around $1500.
Call Cedric for a quote 225-603-2974
ETA I just got a part of my driveway poured and finished and it cost me $8.44/sq ft total.

Called this morning and the cheapest I found was $500 but I have to finish it myself, which I don’t mind at all since that’s not the heavy hard work like pouring individual bags is.
Question:
I don’t need it to be super strong, just going to sit on outdoor furniture on it. Am I ok if I don’t put down some type of rebar or mesh or will that still be required even though it’s just a patio?

I watched these dudes finish my patios this weekend. Yea, I’d pay someone to do that. Unless you have the tools and know the techniques, you run the risk of ending up with an uneven, improperly sloped, water-holding, disappointment.

It's covered so if it's not properly sloped or has a few lower areas it will be fine. Again, I don't need it to be perfect, I just need it to be solid.

I got 12'x17' patio extension for $900. back in 2015.
Guy was going around the neighborhood doing patio and driveway extensions, so having several neighbors also doing worked helped drive down the cost.

I paid $1800 for 20’x18’ with footers back in 2015
Not going to recommend the company because they poured it before a storm. Covered it with visqueen and it cured into the concrete. Took me 3 months of arguing to get them to come back out and re-pour
