I just downloaded the Young Grasshopper workout. Two questions
When to do the cardio sets? I’m assuming right after big four lifts? With or without rest for cardio sets?
Since weight is % of Max, when to re-test max?
the cardio is just more lifting, its immediately after the big 4. no rest between supersets....90seconds or so between rounds.

Looked at the app, how do you figure out what program to pick? I’ve been working out off and on for a few months, fasted 16-18 hours for about 6 months and have lost most my body fat. I’m 155 and benched 220 12 reps last week. Want to put n about 15 lbs of muscle and just get stronger. Just looking for where is good start from where I’m at, thanks.

You're a beginner, so I think most will suggest a linear progression program. I'm not a strong lifts fan, but GZCL, Greyskull, etc are all good ones. Another program that gets zero play here is Stronger By Science (SBS) by Greg Nuckols. It's $10 and it has multiple programs, all are intelligently designed. He even has a linear progression program.
To gain muscle, you're going to have to eat, target roughly 1g protein/lb body weight. If I were in your shoes, I would bulk until 165lbs over 12-14 weeks, then cut some fluff if I felt I needed it
