Is there a free app that provides standard gym routines that I can follow on a weekly basis?
Is there a website that provides these?

Tone & Add definition, lose weight, be more active
*I have a membership at Snap Fitness but my workouts are sporadic and I lack planning. I just go in and do the arm machines one day, back and chest the next, legs next, etc...
Mostly just cycling through the machines but no real goal as to reps or weights. Trying to follow a plan so that I can become consistent and get more out of a day's workout

I use the Strong app to track my workouts.
It has a few programs that come free with the App, but a bunch more that requires a subscription.
It’s a great App that is really easy to use and navigate.
I’d recommend doing some research on a few different programs at LiftVault to figure out what you’d want to follow then use the app to track it.

I use Jefit. You can find a ton of user-made workouts or make your own. You can also have a few different plans that you can switch through easily; I have a 5-day bulking for the gym, 5-day cutting for the gym, 6-day for bodyweight and my Bowflex (thanks Corona), etc.
