Sounds like she’s good candidate for ECT.

She's been on several mood stabilizers. One worked for three weeks and then sent her into a deep spiral. One made her nearly catatonic. Some didn't do anything. She's running out of combinations of meds to try.
Her old shrink retired last year but he was running out of ideas anyway. The new guy has suggested transcranial magnetic stimulation and ketamaine. We're considering it. He's also brought up an inpatient evaluation at the Menninger Clinic in Houston. They don't take insurance and cost $15k a week....

Her current diagnosis is major depression and generalized anxiety but some/most of her depressive episodes have mixed features, ie she can be restless, have trouble sleeping, etc. The only true manic episode she's had since I've known her was caused by medication and quickly resolved when she went off it.
She doesn't fit neatly into one category and her therapists often seem to not know what to do with her.

I've been taking buspirone since June for "anxiety". I can't tell that it does anything but make me slightly light headed about hour 2 after I take a dose. Some call it "anxiety" I call it "I hate people and just don't want to go anywhere that I might have to interact with them." It hasn't helped with that.
I think it is considered a starter drug and it isn't supposed to be habit forming so my Dr. said to give it a whirl.

No matter how miserable she is she can't cry.
Random question: has she tried acupuncture?
My wife started going and for the next 3-4 weeks she was CRYING. Not the normal crying that typically we go through. It would happen at random points throughout the day to where she couldn't even handle her calls during work without breaking down. She swears it had to be due to the acupuncture. The office says warned her that it would occur as well.
I haven't done much research on it to see if that's a normal thing, but your comments about not being able to cry made me think of this.

Funny you mention that, she used to do pilates and it would make her cry once in a while.

This is confusing because I can’t psychically see this. But I some advice. Take it leave it.
Depression with manic episodes is definitely bi polar I know that for sure She will continue to have this episodes if so.
I’d she’s having more episodes I would stop the antidepressant. That makes everything worse.
Anxiety is tough but I guarantee the anxiety will subside once stable. Do not take Xanax. It does not help only helps for a short period. It’s habit forming also being dependent on it. It’s one extra med you don’t need.
The goal is to be stable without of tons of medicine.
Just realize that she is not the first person to deal with this.
It may take some time but with a good doctor it may go quicker. In the end she’ll be alight.
And have no stigma.

I was diagnosed as clinically depressed in 1990. I was probably depressed at least 10 years b4 that. I have recently done 25 ketamine infusion treatments. Its helped some but it wears off fast for me. Success rate is about 70%. Its expensive - $500 per treatment (not covered by insurance). Ketamine works to lower defenses so you can experience on the surface what is causing the depression/anxiety if its related to past trauma. I was so severely abused & neglected as a child that i barely survived. I weighed 120 lbs in HS and i am 6-1. Ketamine will soon become a first line depression treatment option bcuz most drugs dont help much. But you must have a counselor available soon after each session bcuz stuff will surface that suddenly you have to deal with. Its typically 6 doses overva two week period to start. Then if she responds favorably its maintenance after that. Its worth trying. Some patients have had their depression completely disappear.

I know. I wish it was available in the U.S. There are no trials near us unfortunately.

I would not wish ECT on anyone. Its a terrible experience. And it causes permanent memory loss.

I looked briefly. I just hate to spend that kind of $$ if it ends up not working. I will look more into it.

Handful of vitamins plus Ambien for 8-9 hours of great sleep. 7.5/mg of Mirtazapine once a day makes me a happier person.
I’m 69, no heart, BP, Cholesterol meds, nor prostate meds needed. I use only the sleep aids and they work fundamentally as created and they seem to have been made exclusively for me with. O strange side effects.

Also, looks like Silo has set up the first stateside psychedelic retreats, although it's just ketamine.

Judgment free zone. Prozac helps tremendously. Was able to get off klonopin. Still take some others that I don’t want to admit.
I take a SSRI for general and health related anxiety. It has been tremendously helpful with no side effects in my case. Also take ThC in various forms, but that’s confined to the weekends.
