This happens to me whenever I each slide through the ball instead of rotate or I stay on my trail foot instead of shifting my weight.
Both involve moving the low point. Either tee it higher or fix the low point of your swing.

Sounds like you're hitting the top of the ball with the bottom of the club face.

Your ball placement might be too far forward in your stance. Without seeing your swing, tough to say, but this is a common mistake.

Within the first 6 responses you have one guy telling you to move the ball up in your stance and another telling you to move it back. I am not going to tell you one way or another because I haven't seen your alignment but odds are it's going to be a factor.

With the divot.. is it a real chunk of a divot or is it thin?? Reason I ask is because I was doing the same recently and the mark in front of the tee was the ball making contact with the ground because my face at impact was negative because I was not releasing my wrists.. it was almost like I was hit a real squeezer of an iron shot but I was using the driver.. definitely video yourself and definitely find a coach.. Sometimes we just get completely outta whack on our swing and need outside eyes to tell us so

He would have to have an extreme negative AoA to hit the ball into the ground if he's getting the clubface on the ball. Standard driver with ~10 degrees of loft isn't putting it into the ground if you are hitting the ball on the face of the club.
