I was at the doctor's office and in the waiting room were sick people just trying to find some peace, and some dude pulls out his phone and starts playing youtube vids at full volume.

Instances like this are so ubiquitous these days.
The real question: is it worth confronting a complete stranger over something like this?
I could see an argument for either side.
That decision would likely hinge on my blood sugar levels for that day. If I’m already a little irked, I’m saying something.

The real problem is that the restaurant staff should have told the old ladies to end the FaceTime or take it outside, as that was inappropriate behavior for the setting they were in
They chose to ignore the problem and move you to another table.
The behavior will continue as the face timers are still unaware that their behavior is an issue.
People / businesses are afraid to address bad behavior for fear of offending the customer.
