Im looking at the form now. You had to send in a receipt?
And I need to send in a picture of the bottle with the form? I got 20 bottles. They want 20 printed out pictures to go along with the neck labels?

No receipt...unless they changed something with the second offering.
I had 12 that I took the neck tag off. Took a pic of 6 bottles next to each other so you could see the batch numbers on all 6. Stuck 6 stickers on the back of said printout, and did it a 2nd time for the other 6 bottles. Filled out form...profit

I have BCBS dated 9/29/15-anyone have one recently?
