What's your longest hiatus from the game, and how did you fare when you returned?

Roughly 3-4 year break for me and came back 8-12 strokes worse.

6 months after back surgery. 1st round back I'm scared to hit it full. I get on the back 9 one of my partners tells to swing the club or go home.
I hit a pissed off 1 iron and holed my 2nd Albatross on the next hole.
Of course I don't think I even made a par the rest of the way.
I don't think I broke 80 the rest of the year

9 years without playing any golf.
It was a painful return and I'll never regain the skill set I once had.
Now, I enjoy time with family and friends on the course...as an escape from work and other outside pressure.
I have really enjoyed golf more the last few years even though my scores skyrocketed
Simply too much time absorbed by work and family to put practice time in like I should.

Never gone more than 6 months without playing. In HS and college I focused on baseball, but always found time to get on the course. The 2 swings never really bothered me.

I took 6 months off when I broke my left leg last year. My first round back in May, I shot an 80 at Bayou Oaks. It's been a mixed bag since then. 4 rounds in the 90's and an 83.
My iron play, short game and putting is as good or better than before.
My driver and long game has been a problem other than in that first round. I've lost speed and control.

Year and a half after getting done with college golf. Was completely burnt out. Shot 79 and got really hammered doing it. Felt great.

First year or so of law school was a big break, but not terrible coming back.
Took 6 months after the flood off, just because I was really busy. Just recently got back to playing decently well.

Played junior golf from about 5 until 14 and then didn't play in High School or when I was in the Army so i missed about 8 years without picking up a club.
When I came back home and started college I played about 1-2 times a week. Handicap got down to around 3.
Didn't play very much for several years after I started a career and family.
In the last 6-7 years I play about 6-8 scrambles a year for work and I play my own ball 5-6 times a year during the summer. I rarely play during hunting season and usually pick it back up in the spring.
I keep a Handicap online and only count rounds in the last 12 months. It says I'm a 7.9 but I don't know how accurate that is because I really don't have enough qualifying rounds.
ETA: I do have a hitting mat and a flag at about 90 yards in my back yard. I try to keep my wedge game sharp so when I do play, I'm not a total hack. If I have a good day with the driver, I'll usually shoot in the 70's.
Bad day with the driver = Mid 80's

One time I had shoulder surgery and couldn't play or swing a club for 6 months. That's the longest I've ever gone. Due to the nature of the surgery, I had to swing easier with more control. Initially it felt like I was starting from scratch but it actually made me a better golfer.

Very serious nearly fatal heart attack in 2011 forced me to stop playing. Enforced sedentary lifestyle resulted in loss of at least half of my normal strength especially in my upper body.
Fast forward 6 years, I finally told my cardiologist, I would rather die playing golf than live restricted to watching it on TV
Now back playing after a 6 year layoff although only 9 holes. I used to be a 10 from the member tees , now I play the senior tees and still can shoot about 41-42 for nine holes.
I was a long hitter, but erratic. Now that I can'toverpower it my technique has improved and I am pretty consistently in the short grass.
After 6 years of watching the Golf Channel and all the slow motion video, I understand the swing much better and am more sound in my fundamentals.
