My best friend in high school has the last name of Earle. His father's name was Earl Earle.
When I was at SLU in the '80s, a candidate for SGA President was named Felonious Showers.
I know someone who worked at "Old Charity" in New Orleans long before HIPAA. For many (if not all) transactions in each dept, a charity patient would have their "gold card" run through an old school credit card machine that produced 3 hard copies and 2 carbons. That person collected the carbons (that would typically be thrown away) of people with unusual names.
If I hadn't seen them with my own eyes, I would have never believed it. There was a Nosmo King. There were a variety of Indian (feather) names: Hiawatha, Poncahontas, and Geronimo were particularly popular. There was also a "Female", pronounced like tamale, because "the doctor's named her for me."

Oh, yeah...
My ex-sister in law was named Bama, after her grandmother, I believe. As far as I know, neither she or grandma had any affiliation with that school in Tuscaloosa or were heiress to a jelly fortune.

My son's middle name is Artillery, does that count?
For the record, it's a family name. 4 straight generations were named Artillery at one point in our family.
