Not sure if this has been said already, but a more practical situation may be to have two doors, one for the cockpit, and another on the other side of the restroom area, which is about 3 feet away. When the pilot goes to the toilet, the FA closes the second door, and only then does the cockpit door open.
The pilot can access the toilet now, and the cockpit door is by default unlocked, as the 2nd door is now locked. The pilot goes to the toilet, and re-enters the cockpit area, locks the cockpit door, which in turn releases the 2nd door.
It's sort of like an air-lock on the space station. The same protocol can be used for delivering food to the cockpit crew.
PS: As a second measure, the FA can enter the cockpit while the other pilot is in the loo.
I'm not sure that you can do anymore, other than having the A/C be flown remotely from the ground if it comes to the worst case scenario.
You can never totally eliminate Evil, you can just make it harder for them to succeed.

Planes really don't ever crash due to mechanical problems anymore. It's virtually always pilot error.
Air travel is insanely safe. It just comes down to the pilot.

I'm the opposite. I'm an engineer so it would make me feel better if it was a mechanical problem because I know that can be fixed. I don't know how you fix "crazy". That why the Middle East just generally makes me uneasy. Just seems like a lot of crazy over there.

If someone is having major bouts of depression then I don't think they should be in charge of 100+ souls on a regular basis.
That is what I would consider to be mentally unfit to fly an aircraft.

Because in that case at least you need collusion. Yea if multiple members of a flight crew want to down a plane there is nothing you can do.

The CEO of Germanwings essentially confirmed that they guy had a medical leave. German law prevents discussing medical issues, and the CEO said German law disallowed him from disclosing more information.
Or, you can keep believing that muslim terrorism brainwashing was involved.
