What causes "acorn horned" bucks?

I've seen one killed with 1 of 8 points like that. I have never seen multiple points like the above pic before that I can think of.

Caused by injury during antler growth, like if the deer ran into a tree in July. My BIL killed one with every point except the brow tines like that one year.

My grandma killed one and had it mounted that we always assumed was hit by a car in velvet.
The right side was perfect, and the left side was smashed flat and all on plane. You could see it matched the good side it was just smushed perfectly flat and vertical with the side of it's face.

I heard it was infection in velvet but never looked into it.

It looked a lot like this buck, but a smaller main frame 8 on the non smashed side.

I've heard this, and I've also heard that ticks during velvet can cause them. The first buck I ever shot with my bow was a 6pt. I have him skull mounted and he's got a small acorn on one of his brows.
