Agree with a lot mentioned, so I'll add a new one: The Golden Compass.

The Count of Monte Cristo can only be done correctly in a miniseries. There is simply too much detail and too long of a time period.
Anything less sells the story way short IMO.

Dune
I came here to post this. There's zero reason a blockbuster version hasn't been made other than the fact the first one sucked so much it killed the Dune name. It's a top 3 sci fi and a top 3 trilogy of all time.

Movie not even close to the book. Every time I see that movie, it irritates me to say that it's based off the book.
And atlas shrugged. I know its difficult to condense a book that size into 2 hours, but there is so much that gets left out. Like, the animosity between Dagny and Francisco is not broached in the movie, but there's a whole chapter or more devoted to their relationship.

Timeline and Congo come to mind those books felt graphic and intense but they tamed them down to the point that they just sucked.

I don't think k the Bournes where horrible at all. They just don't follow the books at all. The movies aren't bad.
This. Sometimes trying to fit the book into a movie has very bad results, like Unbroken.

Abraham Lincoln plays a key role in the book and is completely left out of that horrible movie.

If you only watched 2 hours of it I would say its safe to say they left some stuff out.

I Am Legend. Movie version goes out of its way to create and slap on some feel good cookie cutter happy zombie-movie ending and, therefore, completely misses the entire point of the book.

Are you asking about which movies change so much that they may as well be original works with the book's name slapped on to trick a built in fanbase or movies that just plain stunk?
By those qualifiers, the best recent result is World War Z, nothing but some names and Zombies give them anything similar...
If I had written the book and they made the movie without my permission, I'd actually have a hard time suing them since it has so little in common.

Forrest Gump.
Forrest Gump is a pretty bad adaption.... but the changes actually make it a BETTER movie than the book.

Have you guys not seen the directors cut?

World war z actually pissed me off having read the book. It wasn't even like the book. The only thing they had in common was that it was about zombies.

I'll say Exodus: Gods & Kings is the best adapted version of the bible. Ridley Scott did fantastic adopting that chapter of the bible into believable reality.
Now as far as a poor adaption, John Grisham's "The Chamber" (novel) was so much better than the movie... Not sure about worst ever though...

Going way back, but I used to be a Stephen King fan. Read the book "Cujo", not bad. When I watched the movie, it was frigging horrible. Just totally ticked me off.
My favorite book of his was "The Stand", that tv movie sucked arse. Both of those really ticked me off. Swore off watching movies from his books after that.
