Ruby Weapon was hard, but Emerald Weapon without Underwater Materia or Knights of the Round was a bitch as well.

Raam on Insane - Gears of War
i think I went hoarse from screaming at the TV fighting that dude. He may have just been cheap though. The line between hard and cheap is often blurred.
Halo 2 legendary might be the hardest gaming experience ever, but I never finished it. I was able to do it in every game that followed in the series, so I assume it must have been pretty fricking hard.
I still haven't beaten Tyson either. I think it's more of a lack of effort on my part than anything. It's a brutal pattern, but I'm sure it's not so bad once you get the hang of it.

Ruby/Emerald weapon by far. Insta-death mechanics are just bush league.

A couple of the Ninjaa Gaiden bosses on the hardest difficulty were just maddening to beat.
Emerald/Ruby Weapon were extremely difficult.
Anyone remember Culex from Super Mario RPG? The semi-secret Final Fantasy-type unbelievably hard boss.
Goro and M. Bison used to piss me off, too.

'super' Lavos during the initial fight in Crono Trigger that you are supposed to lose.
Yes, I grinded to 99 on New Game+ to fight this.

Isn't the outcome of this that he kills crono anyway?

You can warp to fight Lavos at the very beginning of the game if you play new game +.
It'll just be Chrono and Marle fighting Lavos, so it's a bit more challenging. And no, Chrono doesn't die. This has nothing to do with the plot, it's just a shortcut to the end. You get a different ending as a result. It's like a debug mode. You go around to different areas talking to NPCs and it's essentially developer commentary.

Yiazmat from Final Fantasy XII (harder than Emerald or Ruby by far).
Demi-Fiend and Satan from the Shin Megami Tensei: Digital Devil Saga series, and Matador from Shin Megami Tensei: Nocturne.
All of the above trump anything on that list.
