Looking for stomach area pain diagnosis.

Looking for help here.

45 yo male
Year long intermittent dull pain in area between left hip bone and lower left rib cage. Can’t pinpoint when it happens. No clear food cause. No clear position cause. Never changes pain level. Either the pain is there or nothing.

Endoscopy negative
Colonoscopy negative
Tried Prilosec - nothing
Put on a low dose ssri to try to interior nerve pain. No help
Ultrasound of kidneys negative.
A year and a half ago, have laparoscopic surgery to remove kidney stone. No kidney stones visible and blood work is clear.
Did PT for a while for lower back pain and hip misalignment. Didn’t touch it.

Any help here would be great. The pain is not debilitating but is almost constant.
I’m not sure what route to go next.

Interesting- I have something similar. 40, active- started a few months ago- sometimes it’s there and sometimes not- I think it’s muscle related- sometimes sitting with a pillow next to it helps- from my online research- I guess muscle- if I’m actively doing something- working out or biking, I don’t notice it- usually when I’m sitting
Also there seems to be some rib cage abnormalities like slipping rib or something I may have because sometimes it’s on the rib cage- not debilitating so I’ll just deal with it

Almost exact same for me.
Sitting driving is when I notice most.
But zero pain when I am doing physical labor. I figured if muscular, possibly would get worse while lifting and such.

I know it’s a long shot but have what about shingles?

Shingles can present with just the nerve pain and not a rash. And the post herpetic neuralgia can last a long time.

You’ve had a pretty solid work up and that could be a last ditch effort. Ask your doctor for Amitriptyline 10mg at bedtime.

Side effects are pretty minimal but can cause some urinary retention and blurred vision. Just a thought.

I’d also like to do a hip exam looking at a possible labral tear. Again another long shot after your work up.

I tried Amitriptyline I think 25mg and I did have some mild depressive side effects so I stopped.

Shingles maybe but I have had them before maybe twenty years ago and I don’t think that’s it either.
Is there a way to test for shingles that is rashless?

ETA: Also having a lot of gurgling in my stomach over basically the same period.

I’m not actually sure offhand. If the amitriptyline didn’t help, then it’s likely not shingles.

I too did some physical therapy for lower back pain and talked to the PT about it- he didn’t seem concerned- I find if I don’t think about it, it goes away- he explained it not hurting during physical work that the muscles were getting more blood flow
I first noticed it while sitting/driving- honestly felt like a slight discomfort that I’ve felt before through my life but would always go away- I assume it’s muscular or some strange structural abnormality that’s finally showing up-
this may sound strange, but ever since I was a child, if I lay on my right side, and pulled my left knee up, it felt like my hip was hitting my rib and it was uncomfortable- so I just figure it’s related to that -

Give this a try:
-Lay on your back with your knees bent.
-Place a small softish ball (Tune-Up Fitness is my favorite) on a section of your stomach. On top of that ball, hold an upturned 20lb dumbbell.
-Using the dumbbell like a joystick, apply pressure, relax and let it sink in, and gently massage.
-Start just above your groin in the inside of your pelvis and trace up to the outside of your hip. Hunt all over, but the primary benefit is hitting the illiacus.

Sorta hard to describe, searching “gut smashing Kelly Starrett” might help find some old videos he did for a few other methods

A lot of what everyone on here is talking about sounds like pain caused from a tight QL (Quadratus Lumborum). They often tighten up due to positional tendencies caused from low back pain or tight Piriformis muscles.

Try working on releasing/stretching your piriformis. Followed by a few sets of clamshells. Stretch the piriformis again. Then move to QL stretches. Do a few sets of QL exercises (side bending) focusing on the stretched portion of the muscle. Stretch the QL again.

Don't forget to stretch Quads and Hamstrings as well, as they are major contributors to low back pain/dysfunction.

Try a foam roller.
Concentrate on the back upper shoulder area.
