For those who have been, where did you go, what cruise line, and would you recommend?

Ive been on two, would recommend but recommendation would vary based on if you were bringing kids or if it was just adults.

I have been on one. It was round trip from Rome.
We had a lot of fun but I will say this:
A Mediterranean cruise is not the best way to experience Europe for several reasons. For one ,they don’t spend enough time in port to allow you to fully enjoy the cities, especially the restaurants at night.
Also, many of the destinations are a drive from the coast. Rome and Florence . are two examples. Same with the Amalfi Coast or the places you would want to go while docked in Naples.
The French Riviera and Monaco were good stops. And Palermo was pretty good.
We were supposed to go to Tunis in Africa but we couldn’t dock because of rough seas.
We are taking one next year to Greece, the Aegean and the Adriatic. Looking forward to that.

We did the Sun Princess from Barcelona to Rome. It was one of the first voyages for the new ship which I believe is in Caribbean now. We enjoyed it, but there was only like one sea day, which if looking I'd recommend one with more sea days. Also a lot of the places we stopped required long trips for excursions...Marsaille, Genoa, etc. Our first stop was in Gibraltar which was ok. If I were to do again, I'd do from the Greek side ending in Rome.

Did one on Cunard a year and a half ago. We started and ended in Barcelona and went to Naples, Civitavecchia, (near Rome) La Spezia, (near Florence) Ajaccio, (Corsica) Toulon, Palma de Mallorca, Ibiza and Valencia.
It was fantastic. I would definitely go again. Every place the Med cruise went was beautiful. The downside is that I am now ruined for Caribbean cruises.
As someone mentioned above you are not going to see a lot of the cities in the short time you are in each one. Think of it as a scouting mission.

Wife and I and another couple did the Viking Empires of the Mediterranean. It was great and we may do it again.

We’ve been on Seabourn the last 3 trips. Highly recommend. Smaller ships -225 cabins and 300 cabins. In 2023 the itinerary took us from Barcelona to Menorca, Sete, Bandol, St Tropez, Malta, Trapani, Lipari, Taormina, Amalfi, and finished in Rome.

Pick a couple of cities and spend 3 days in each, much better way to visit.

My mom died in April so I booked a last minute Greek Isles cruise on Celebrity to get away. I wouldn't have even looked at a cruise for Greece but the timing was right and I didn't have to worry about planning things. We went from Athens to Santorini, Ephesus, Rhodes, Mykonos, and Hydra. It was honestly a wonderful experience. I would do it again, maybe trying a smaller more luxury line, but Celebrity was great. Not too big, not too small, nice fellow passengers, and great service.
