If terrorists do want to attack a small town...all they have to do is install a few roundabouts on roads overnight.

I think medium sized cities are at most risk. Large enough for high numbers of horror. But not over-prepared like NYC, CHi, etc.

If they wanted to cause real panic, chaos and fear, that is precisely where they would do it, random small cities around the country. In the past, they seem to want to inflict the most damage with the least amount of work or risk of being exposed prematurely.
