When I first became a parent, this was a fear of mine as I seemed to see news stories about it happening all the time. Several years later, I now don't know how that possibly happens. The kid should be the first thing removed from the car.
edit: and before the naysayers come out, in all of these cases, you'll probably find that they remembered to get their phones and wallets/purses.
