This used to happen to me when I was a kid playing baseball. I honestly never knew there was a term for this (the "yips") until recently. It is hard to explain and frustrating as hell. Happened to me playing third base.

I played with a guy in high school who got the "yips". He could throw down to second or third or first or anywhere just like normal. Then one random day for some reason he could not throw the ball back to the pitcher. He looked just like this guy. He kept doing it for like a couple weeks. He cured it by throwing balls from the mount to someone behind the plate. For some reason, that cured him. The yips are a strange phenomenon.

also reminds me of Darrin Dalton of the Phillies, I believe, who struggled throwing back to the pitcher, and had similar hitches in his motion.
