I think it’s a combination of failing infrastructure and lack of proper maintenance due to labor shortages or incompetent labor

You are staggeringly naive. Accident isn't summarily ruled out, but given the events in OH in the month of February this year, only the most ardent of windowlickers is going to go straight to the #MuhIndustrialAccident conclusion.
