Never been asked. I have a picture on my phone and that's either good enough or I'll go somewhere else. They made it too big to fit in my wallet and I don't want to risk losing it.

Mayor of NO issued a "dictate" requiring vaccine proof in restaurants and bars. There's not one statewide, just the stupid pointless masking requirement to walk to a table.

I worked at a number of fast food places growing up. I promise you that is the mindset of 90% of the workforce. Whatever results in less work is what they WILL do. Especially if they can legalistically do it within the framework of their job.
These are the type of people who work harder at trying to figure out how not to do work than they would work if they simply did their job and went home.

My question is, do they have some type of scanner to verify it? Do they call the doctor's office and verify it? Is the vaccine card a universal look? Would they know or care if you just flopped out a piece of paper?
Make sure you let the franchise owner know you took your business elsewhere. The employee doesn't give a shite, the owner will if they see it hit the bottom line.
I went to Popeye's for lunch, and it was drive thru only, and it was 20 cars deep. I said frick it and went to one of the other 100 places to eat within a mile. Business decisions have business impact. I can say for sure in the 1 minute I was there, I saw several cars pull in and drive out when they realized the situation.
Drive thrus may be great, but they are not super efficient when the 8th guy in line just wants a coke and fries and the 4th guy is ordering stuff they have to fry, etc. Sometimes people want to sit down to fricking eat.
