My take is that they try *really really* hard to believe it is real and when they come across daily challenges to this fantasy (like getting a pee dribble on their zipper or having their stubble grow back) they get super frustrated. Which is why they try to force their delusions on everyone around them.
The fact is that a man can never know what it feels like to be a women (or vise versa) because a man can never actually be a woman. So their statements about "feeling" like the opposite gender are a fallacy from the get go

It makes people that don’t feel special now feel special. Much like self loathing based on race/religion. A below average white guy that can’t stand out among his peers now stands out to the almost-majority minorities by denigrating his own kind. It’s so easy to do
