Fish, I know this is not the thing families are always prepared to hear. Please, take these words with the heartfelt sentiment in which they were written.
Changing the treatment plan and care goal is not the same as stopping treatment or care.
Changing the plan to most accurately benefit the patient's desires and concerns should always be the most important goal. If that goal is to have as many days as possible on earth regardless of risks or potential complications of treatment then that should dictate the plan of care. If the Goal however, is to spend as many days as possible relatively functional, able to communicate with family, and relatively painless while at home or not in the hospital then the treatment plan and goal of care needs to be adjusted accordingly. Sadly, it is the excessively rare situation that those two wishes are mutually attainable when the disease is at all serious.
I have seen too many patients who have been victim to unwanted pain, debility, and hospitalizations due to decisions made from selfish desires. Sometimes it is due to the patient not wanting to appear to be a quitter to their family, someone not wanting to disagree with the doctor, or a patient's family not wanting to lose a loved one.

BP, I agree. I had to have a sit down with my brothers and sisters due to having POA on our Mom. It wasn't a easy thing to do withholding treatments from a love one. Even with a team of doctors answering all questions, it still ate me alive. Still does to this day, but I knew and my brothers and sisters knew it was the right thing to do for her.
I should see my brother maybe this weekend and I will have that talk with him.

Not withholding, changing treatment. Pain meds anxiety meds, social and emotional support are all treatment. You just change from trying to prevent and cure to trying to comfort and support.
Regardless it is an unnatural response for most all of us and very difficult to process and be a part of.
Regardless of your brother's or your family's decisions, I pray for his continued comfort and fulfillment of his desires as well as for your health and wellbeing.

That is great.
Caught more fish over the years out of pirogues then all of my offshore boats in the past.
