It can be done, keep in mind that it is probably a riot type shotgun with a short barrel and not your 32 inch goose gun

Pulling the shotgun out of the truck or bed of the truck and it's loaded. Trigger gets snagged and it goes off. I think there are a ton of way something like this can happen. Gun could be dropped while loaded and fire, etc. I really don't know how this happened but there are a lot of possiblities with a gun. Maybe it was a pistol grip shotgun.
