Had recall work done on the rear axel of my car about 7-8 years ago. Drive a few blocks down the road towards the interstate. The whole car started vibrating heavily. Got out and noticed that one of the wheels was loose.

Slowly took the back roads back to the dealership. Of course they blamed it on the new guy.

Honda called with a survey a few weeks later, and I left poor reviews. I received a very angry call back from the service manager the next day. He probably got his arse chewed from corporate and wanted to take it out on me. Needless to say I never returned with my business.

Where I bring my truck for a tire rotation the tech walks out with a torque wrench and checks every lug nut after he takes it off the rack and before you get your keys.

Yes I had this happen to me yesterday after having my lifted truck serviced with a 150 point inspection they done a oil change and the tires were rotated and balanced after i noticed the steering was loose then my drivers side tire flew off hit another car and Thank God I didn't get hurt and the other driver my 35 inch tire hit their car they didn't get hurt as well but my truck is really messed up and slid down hill a long ways the wheel came off with the hub cap still on that's why pictured it laying next to my truck.
