Dillard's Gold Label, I've worn them for 20 years and they are truly wrinkle free, long lasting and come in a huge variety of neck/sleeve sizes for about $65. The current sale as mentioned a page or two ago at $35 is a great deal.

Man, they have went up a lot if they are $200 now. The last ones I bought 3-4 years ago were $135. White, frenchcuff, made to order.
They msrp for $198 I think. I never mind paying what is asked because the people I go to tailor everything they touch and will make sure it fits right. You can usually find a couple on sale if you look hard enough which is what I do every once in awhile.

Dress Shirts is the one thing Jos A Bank does pretty well, especially for the price when on sale. Wait for a 3/1 sale and you get them for around $30 per.

Charles Tyrwhitt or Paul Frederick. Both have good sales from time to time where you can pick up the basic business white dress shirt and customize your collar style and keep it in the $50-60 range. Monogram is usually about $7 extra. They both also have some nicer shirts in the $80-$120 range for your dressier days. Wide range of sizes as well. I wear a 17.5/38 and both have shirts that fit me.

I've 3 or 4 of these and they are by far my favorites. Also have a VV shirt that I really like, but it's a button down collar, so can't wear with a tie.
JAB does make some decent dress shirts.

Can't beat Van Heussen for the price. I like their wrinkle free line, and I have a few that have stayed nice looking for years.

I feel your pain. I struggle finding 16.5"/36" shirts (and avoid the Big and Tall section, as there is no Big or Tall option, sadly). Apparently either everyone that's my size gets there before me, or most guys with a 16.5" neck have really narrow shoulders and short arms. I'd imagine your 38" sleeve has to be special ordered/tailored more often than not.
