Jet Airways and Aviataca are tied for the lead in my book.
Jet Airways is a special kind of flying hell, I was the only person who had likely showered in weeks and no individual meals, just a curry cart comming down the aisle ladeling shite into paper bowls.
Aviataca actually had livestock (chickens and goats) on a flight from GUA to SAL, though that was quite a few years ago.
Anyone complaining about the US majors needs to get out more often, they are among the worlds best with Delta being my favorite.

The one that goes between BR and Ascension. Its the worse.

I am platinum with American and I have been Platinum with Delta so I fly pretty extensively. I have a 50/50 split between domestic and international.
Since I am American, I stick primarily with Oneworld airlines. (I did like Delta, they just did not service where I went).
British is the only airline I have had an issue with, and partly is was not their fault. LHR was shutdown due to massive snow, but how they handled it was pretty bad. Basically said go home, we can't help you here.
Let me start off by saying, airlines cater and only care about elite flyers. Something like 75-85% of all of their revenue comes from flyers who fly the airline 2 or more times a year. With that being said, if you fly often, you will eventually deal with: delayed flights, canceled flights, mechanical issues, weather, etc. You can only fault them so much for these things. I would rather be on the ground with an overpriced beer and shitty sandwich than dealing with it in the air. Any time of flying issue, I have dealt with. I will say AA has never once lost or damaged (beyond normal wear and tear) my bags. My last trip to MAD, ORD was getting snow so before I even called them they changed my flight to go through DFW and got me there and my bag on time. Little things like that matter.
I have never flown Spirit/Frontier/Allegiant but you get what you pay for with those guys.
I did not like Iberia, but it was not bad. Sun-Air of Scandinavia (BA subsidiary) was actually very pleasant on a Dornier 328. Flew them from Billund, DK to Dusseldorf en route to LHR.

I don't understand the hate for this one, It's a pretty straightforward concept for a direct flight at a seriously inexpensive price point. For weekend flights it's a great deal, and for two hours out of your life it's not even that uncomfortable.

Sw but only because of the seating system. It makes flying with anyone else a real bitch.
