perhaps the worst recent Mass-Murderer: Andreas Lubitz

three years ago, at this very moment, Germanwings 9525 was supposed to land in Düsseldorf. Instead, it was deliberately crashed into a French mountain by co-pilot Andreas Lubitz.
Of the ways to die, those passengers had it pretty bad... the terror of hearing the pilot bang on that door, louder and louder as the mountains got closer.

Because of that, I always get anxious when one of the pilots leaves the cockpit to use the restroom.

Most airlines now require an FA (or other crew member) in the cockpit if the pilot or co-pilot needs to leave. Is this failsafe? No, but certainly puts a lot more obstacles in the path of a pilot-suicide/homicide situation.
