It was a database established years ago. Physicians had the option to "opt out" of being included in the database. My reps have jokingly complained to me that they can't see all my numbers but the can track the discount coupons that I hand out to my patients so they know I am writing SOME of their product.
I do not remember who made the point before, but the biggest influence in the products I write every day is INSURANCE COVERAGE on the drug. If Blue Cross makes it their three, you won't see me write it much due to patient complaints of cost.

What did you want him to do, stare at your acne for 10 minutes appreciating how unique yours was? Review history, check lab work if needed, prescription and follow up. That's all that's needed.

Don't ever go to a MEDAC unless you need general stitches. I have noticed a lot of PAs out there. Docs are so busy it is hard to get in. There is a shortage but no one wants to admit it and it will get worse.

There was a time back around the mid-90s or so where insurance companies tried to make a big push into dentistry with the whole PPO and fee-for-service plans, but dentistry resisted this. If there was a major employer in your area that was on a plan, maybe you made an exception to keep those patients in your practice, but as a whole dentistry fought back. There was a dentist here or there that panicked and accepted every crappy insurance plan - and they made money if they were the only guy accepting it in their city - but they had to bust their arse and see twice as many patients (which meant crappy dentistry). Dentistry simply did not allow insurance companies to call the shots, and it's a damn shame what they've done to physicians and healthcare in general.

Physicians like to think their profession is financially superior to dentistry when looking at median incomes, but it's comparing apples to oranges - you guys have to bust your arse to earn a living. A dentist can live very well seeing 10 patients a day 4 days/week. Hell, once your practice is churning and you toss an associate in there along with a full time dentist, you can make a damn good living working half that (my last couple years I was forced down to 16 hours/week by my neck issues and still did very well).
If you want to compare apples to apples, the dentist who originally owned my practice busted his arse and saw 30-35 patients/day and took home $450K/yr. in the early '90s working 32 hours/wk.

I for one feel that it's very difficult to find really good medical care. That doesn't mean that good medical care doesn't exist but that not all doctors are created equal. Different doctors will provide a different diagnosis or will present alternative means to solving a particular problem.
I had a minor health scare in January and my experience has been that doctors are too quick to write patients off based on age or a variety of other factors. They are also far too quick to throw prescription drugs at symptoms rather than to solve the underlying problem. You get the impression that they are trying to usher you out the door as quickly as possible. Once you have paid them, good luck getting in touch with anyone to answer any questions or to inquire about additional symptoms that may not warrant a doctor's visit.
