Usually at least 2 people monitoring everything, locked door when MRI not in use, markings on the floors outlining zones... they take it pretty serious.

However, if some dumbfrick goober decides to run into the room then there's really not much they can do.

Sure you can with card access. Doesnt have to be literally locked with a key. The control room entrance should be locked

I googled "items that weight 20 lbs" and here is a list.

Not even necessary. They have multiple "zones" and someone's relatives should have never been that close to the door.
