Reports out of UMC in NOLA are that 40% of patients going on vents are coming off.
So not great.
Hospitals are very concerned about PPE and staff getting sick and running out of staff. Intubation mitigates that risk. As a patient, lay on your stomach and cough like a mother fricker, avoid intubation.

This is the only time in my life where even if you aren't "DNR", you are.

300 pounds? BMI >40.
This seems to be the most reliable risk factor other than age. Not sure if its due to poor clearance of secretions due to body habitus or if there's underlying OSA leading to increased pulmonary pressures and that makes them high risk.

P/CC doctor I talked to who has seen the most success is doing the things the evidence has always shown in ARDS... high PEEP, low TV, prone positioning.

Mask is a small part of it, meticulously washing hands and not touching face is more important. If you wear a mask, it will live on that for hours to days, so when you take it off you have it on your hands.

Regular masks have evidence to suggest they help block an infected person's virus droplets from hitting the open air. So good for symptomatic people.
Not much evidence to support prevention from getting it. So not great for asymptomatic people.

My wife who works in the PACU is getting transferred into the ICU to treat CV patients and she’s scared shitless....and I’m beginning to be.

anything she can ask/demand before her new shift starts on Monday?

Date from china suggested they were bad, but this is likely BS. Some experts believe they are protective. General consensus is more harmful to stop them if already taking than to keep taking.

We use it all the time. Long term for lupus and other connective tissue diseases, sarcoidosis, and recently for severe osteoarthritis. In the OP's defense, he's an ER doc and has never prescribed it or managed a pt on it.

One thing that needs to make a comeback is mucomyst via nebulizer for those thick arse secretions. Nothing works better for breaking that shite up.

Real cure would be to stop intubating people who only need high flow O2

I'm betting many could be on Bipap to help reduce respiratory fatigue. If started soon enough, can keep many off vent.

Zero. Carry on. Stay away from elderly.
