Family of four
HSA
$715/month.
Company picks up the rest, which is about $600\month.
$3500 deductible per person
$6000 deductible per family
100% you pay everything until you meet the deductible, idk. Never reached it.
No co-pays but you can have a pre-tax payroll deduction to a credit card to use in paying the deductible. Sucks.

Wife and me $498/month $5000 deductible. Son individual $250/month no deductible just copays. Baby due in 2 weeks will have the same. $1000/month or a $200,000 house note, with ins. And taxes paid. Makes me sick to my stomach to think about, but I won't go see the Dr. die to the 5k deductible.
