Yes, when you ask for the straw and she gives you a dirty look.
Once asked 3 times for silverware and finally ended up sharing with a family member. I paid the bill, and on the tip line I wrote "zero, thanks for the silverware".

Bacteria can go through 13 layers of single ply TP. That paper wrapper isn't doing you any good. Besides, you touched the menu which is probably the dirtiest thing outside of the bathroom and maybe some door handles.
