About every 25 brushes, so once a year or so.

I replace them every February or March with those big toothbrushes thrown at Mardi Gras.

I currently brush with an oral-b electric that has a little alarm when I brush too hard….so I can go 6 months without changing easily.
However, when I was manual brushing, I probably changed out every 3 months, but needed to change once per month.

I change the brush head quarterly.

Hard bristles, not very often, medium bristles every few months. Actually something between medium and hard would be perfect for me.
