Also, is it like a chunk of sausage, like say a 2-4 inch length of sausage? Some places serve their beans and rice like that with the sausage sort of on the side like that. It's pretty standard.

I mean you got red beans and rice. I would chalk it up to bad luck and just not order it again.
Now a severely under cooked or over cooked steak? Hell yes. I would send it back.

The only times I have complained or just asked for some changes is in higher end restaurants. I'm not eating at a chilis or somewhere and complaining about the amount of an ingredient in whatever I personally selected.

My father in law does this all the time. He complains about food and service. He doesn't do it as much when I'm with him, because he knows I hate it. It's just how he is. He tries to get a lot of shite for free. He's very well off, but it's like a hobby for him. Polar opposite of myself. If I found a hair in my food, I would probably eat around it.

Once had a burnt steak when I asked for Medium Rare. I see no reason why I shouldn't have told them to take it back and get me salmon.

Only time I did was at a well known BR restaraunt. Not really a high end place but good enough to not have this happen. Don't want to say the name because it's one of my favorite places to eat and this was an isolated incident.
I ordered tuna and when it came out, the inside was literally still frozen. I could see ice crystals in it. They took it back and said they were going to cook me a new one. The second wasn't frozen but it was still cold in the middle. Sent it back again and refused to order anything else. Ended up getting a table for 4 with drinks completely compped.

I've worked in the service industry for a while, so here is what I think. If you go to eat, and something isn't right with your food, you should say something. It's not cheap to eat out, so why stay silent eat a meal that's not enjoyable?... The other end of the spectrum is people that eat an entire meal, box up any left overs and then complain after the check is dropped off.
