One fix is to put a mock-up of the emergency exit at the gate. If you cannot get through, then how would you get out in an emergency? If you could not get out then you should not be allowed to get on.
Just consider the situation where an emergency is taking place and one of these people clogs the exit or route to the exit for all others behind.
And then there are the inflated ramps to consider.

First class seats are generously sized. A person would have to be quite large to get stuck in one.

They should have left their fat arse in the seat until they lost enough weight to get out on their own. Only give them water. They’d get out in a few days.

Pretty disgusting that airlines subjected this person to such embarrassment. All because they have to reduce seat sizes to maximize profit

If you can’t handle her at her seat stuck fattest, then you can’t handle her at her well, wait, there is probably only ever gonna be a seat stuck fat

I mean, if you could get him IN without removing the door, wouldn’t it stand to reason they could get him OUT without removing it? Did he somehow gain 100 lbs on the meals in first-class?

Shooting from the hip here, but judging from the commercial planes I've been on, there is an overhead compartment above the seats.
I suspect this morbidly obese person once seated did not have the core or arm strength to stand. People trying to aid this person would also obstructed by these compartments to lift much weight in a crouching position.
By removing a panel or seats in front of this morbidly obese person, they might pull them forward and back on their fat arse legs and swollen diabetic feet.
