I had a Yakima box ( I’d bought used in a pinch —not smart for cross country trip unless you rrallllly inspect it) hinge fail and thing popped open an the contents flew directly into the old couple behind us on I65.
Thankfully it was all pillows and soft baby stuff. But still we all stopped, and I made sure they were alright. I felt horrible and it has left a mark and reminder to be more cautious.

I had a sleeve in my draw hitch I forgot to take out when I pulled the hitch itself out.
That have metal sleeve came out at some point and all I could think about is how bad it would have fúcked up someone behind me

I saw a video of a car driving behind a trash truck. A brick bounces out and crashes through the passenger side of the windshield. It was the guys wife. You don’t see what her head looks like, but you can hear the agony and hopelessness. It’s one of those videos you wish you wouldn’t have seen.
Keep your distance on the road. You’re not getting anywhere much faster. Give yourself time to react.

Man, seems like the driver is somewhat responsible for the death. If he left the pin out and the hitch fell off and went through windshield, yeah, seems he could be at fault.
Not if he had a "Stay back 500 feet" logo stamped on his tail gate.

What a nightmare. A few years ago, a truck lost a tire along I-10 near the MS state line. It bounced all the way to the La welcome center, where it collided with and unalived a young woman in the parking lot.

I have a lockable round bar that keeps my hitch attached
A simple cotter pin through a hole on the bar is such a poor design.

I have a commercial grade HD bolt through mine and I leave mine in at all times I’ve been rear ended one time and mine went into the radiator of the truck that rear ended me but it stopped the truck from damaging my truck. Was going about 45 mph on 190 between new Iberia and Lafayette.

I was driving in the middle lane of a 3 lane interstate, properly spaced behind an 18 wheeler, and some asswipe cuts in between us from the left lane and ends up hitting a 2x4 lying in the road and kicks it up and hits my windshield on the driver's side.
Luckily for me, it struck enough of the A pillar to not come crashing through the windshield.
It did, of course, crack the windshield enough that I had to drive like Ace Ventura for a few miles till I got home.

Somewhat? How about totally.

Did the driver know it came off his truck? And did he stop?

Several years ago I was following a Pick Up, pulling a dual wheeled trailer on I-59. We started down the side of a mountain between Gadsden, Alabama and Chattanooga, TN. Then the hitch failed, and the trailer was rolling free for a hundred yards or more. I suppose they never connected the safety chain. I slowed and watched. Then the trailer made right turn, off the shoulder and out through the trees.
