That's not necessarily true. Not for profit hospitals are mostly a racket to avoid paying taxes. I work in healthcare advertising and one of my accounts years ago was Northshore University Healthsystem in the wealthy suburbs of Chicago. They were not for profit because they "reinvest" their profits and provide a small amount of free services to the community. It had around 4 locations and the CEOs salary was about $2.5 million.

Pay in rural areas last I've seen is actually good? That's one of the reasons I've seen given for the high cost of rural healthcare, is they have to pay more for the professionals to work there because the demand for the job is low.
Needless to say, as said this wasn't Alaska, Wyoming, or West Texas. It was Connecticut. Its not really that rural anywhere there.
