IMO the larger issue is how Massachusetts determines their "not guilty by reason of insanity" plea. The duty is on the prosecution to prove sanity at the moment of the murders, not on the defendant to prove they were insane. Which is more difficult to prove IMO.

There’s a distinct difference between someone who is psychotic or a sociopath and someone who is manic.
My family member has had a few manic episodes under certain circumstances, and was completely out of her mind. And needing to be restrained in a metal hospital until she could get back in a functioning state.
There was no hiding her behavior and state of mind. She was not capable of plotting or scheming or manipulating someone into something like leaving the house for multiple hours
