I personally witnessed something very similar to this about 12 years ago. Drunk friend laid down on his back holding a mortar tube while another lit and dropped one. It went in upside down and exploded in the tube while he was holding it. I looked like a med school cutout of a hand - palm blown completely open and you could see the muscles and tendons moving.
