I've said a few times here that these people can only do these things because they are afforded the comfort and luxury of doing so. They simply cannot exist in the form they choose without the continuation of modern medicine and drugs they are required to keep propping up that chosen "identity".
Let shite go sideways, let all that medical care melt away, and they revert back as close as they can get to their natural form. We can't defeat Nature. We can manipulate it to an extent because of our medical advancements, but it is still Nature.
People making these decisions are removing themselves from the gene pool. Naturally speaking, they now serve no purpose to the survival and advancement of our species. The comfort of our current society affords them the opportunity to continue living and contributing to it, but Nature has no use for them.
That doesn't matter as long as the medical community and infrastructure is around to continue propping them up. However, in a world that can no longer afford that luxury, their fairytale world would come crashing down.
ETA: This makes perfect sense if you prescribe to the belief that we are all animals. Our Natural, instinctual base is one of animalistic survival. Our advancements in technology and medicine have removed us from that base to an extent, but if a person refuses completely to acknowledge that base, my point will find no home with them.

Newspapers articles, media reports, TV shows, movies, etc.
It is present in everything.
It is present in schools, which is probably the number one reason why it gets such a loud response.
Something that prevalent will of course garner a loud response.
