Today I noticed the master bathroom mirror is disconnecting from the wall. It's separating at the top and is leaning.

Does the whole mirror need to come down and be reattached or can I just put some mirror mastic on the top to reattach?

TIA

Have your wife/gf stand in front of the mirror naked and take a pic. We will then be able better advise you as how to correct the problem.

Take down and rehang with mastic. You could try and squirt some mastic behind there with a caulk gun, but mastic is messy and you run a high chance of getting black tar all over the top of your mirror. You could use clips, but meh, those are ugly.

Take the whole wall down. Don't go the easy route. Will have to be done eventually.

Mirror was secured properly and house is only 7 years old. What would cause it to fall?

Humidity in the bathroom from the shower or bath.

ETA: Or the house's foundation is settling.

Lowe's sells the mastic that comes in caulk tubes, near the mirrors in Home Decor. I've bought it there and used it several times. I would take the mirror down if it will come off easily and reapply the mastic.

Does that involve sanding down the old mastic from the wall and mirror?

I am assuming that there is no frame around the mirror and that it is not currently held by clips, correct?...
