Well, it comes from large populations of Irish, German and Italian immigrants all speaking English to each other as a second language with their respective accents that merged, to varying degrees to form the Boston, New York/Northern Jersey, Philly/South Jersey accents. Similar influences were at play in the Midwestern cities, but you had more Poles in Chicago and Ohio, more from Scandinavia in the upper Midwest which color those accents differently.
The only comparable city to those (particularly Northeastern) Yankee cities is New Orleans and that is why its accent is unlike anything else in the South and more like those Northern cities, although the French accent colors it in a unique way as well.

The YT vid says "Clay" called in to Stern on August 13, 1997.

"On August 4, 1997, he was arrested on drug charges and was incarcerated for 85 days."
If these dates are accurate, he would have to have been calling from jail, which seems unlikely.

People can say that, but I know a native New Orleans accent and that isn't one. That guy is from the east coast.

Funny, because it sounded to me like he has a lot of northern Illinois/southern Wisconsin accent in his voice.

The guy identified himself as Clay, mentioned if he killed himself he would miss the Batman movie, says he kills with a hammer, talks about comic books.
