The bank I worked for started doing this. The first time around they published the comments, and people were just blasting the company.
Every time after that they stopped posting the comments.
I wouldn't trust being 100% anonymous on those though.

I’m not sure if the comments are anonymous. I believe the 1-5 ranking is.

Our former COO did that. Wasted about $500K plus lodging and travel costs for a team to come in. First thing they did was send a form to drop off in a secured box with things the employees suggested. After a month of "investigative research", they compiled a report and literally brought the employee suggestions to board and then billed us later that day.
The board had already reviewed employee suggestions and when they saw that the report basically compiled the suggestions into a document they knew they had gotten taken.
About a month later that COO was no longer employed by us.

We had one at work. Completely anonymous. Ran into my boss a month later at a bar in Destin. He was drunk and all he could talk about is why I wrote this and that on my survey

I worked in Management in Healthcare and we were not given any details on who said what. The only way you could be identified is if you said something in the comments section that gave yourself away.
That said, I always understood the concern.

I get these all the time and I give it to them straight just as I do my boss face to face. They can 100% tell it’s me even if it really is anonymous because I say the same things I tell them in person. I don’t care, I haven’t been fired yet and the same things I complain about are still present. frick it.

I think there are other factors too that can give you away too. For example, the survey response might also indicate what position you are in at the company, and how long you have worked there.

Coworker was raising hell on one of those surveys. They don't know who is filling it out! I told him to answer one of the questions by saying he was going to come to the office and shoot it up. You'll find out how anonymous it is.....

We completed a survey that was submitted to us to complete about a month ago. I’d be surprised if they every report the results
