If you look at studies conducted by the NIH the US develops less than half of all New Molecular Entities (pharmaceuticals that had no previous approval).

What we have now is state capitalism: a form of economics that Marx himself said is a needed precursor to full-blown socialism. We are by no means a free and open capitalistic system anymore. Covid gave us a preview of that when the politicians and bureaucrats said mom and pop hardware stores weren't critical, but Lowes was, etc., etc. It decimated small businesses, further lowered any competition or profit drain from the corporations that are locked hand-in-hand with the government, and they raked in record profits in the process.
Once that damage was done, *tah-dah*, here's trillions in fake, printed money for socialist programs created by the damage done from the government, and the obvious outcome was it did nothing but cripple us even more and lead to massive fraud. Rinse and repeat.
Classic Cloward–Piven strategy.
If there's one thing that could sum up the Biden administration's legacy, it is that they excelled at pushing all 3 major tenets of the Cloward–Piven strategy in order to further move towards full-blown socialism. We have long since crossed the Rubicon headed towards socialism, and socialism makes sense to so many because not only are so many so poorly informed, but they also see Great Britain, etc. getting "free" healthcare while we fall into financial ruin after breaking a leg.
Aside from land size and population differences, what they fail to understand is that the precise reason for that discrepancy is because of socialism: They take from us to pay for others who didn't contribute. It looks great because we're the ones enslaved to pay for everyone else. The end product is communism where we are all enslaved with all of our outcomes being a shite sandwich of death and despair.
Go down the rabbit hole of how we Americans subsidize the rest of the world's healthcare — not just from eating the R&D costs, but also through direct subsidies and more— and it will enrage you (unless you're one of the ones who got pissed off at the messenger for showing you a taste of government fraud, if that's the case then you've no hope for common sense and logic).
Fall and get a compound fracture in this country and you're financially burdened for life, and most of that has to do with us covering the cost for the parasites who get it for free while spitting in our faces: Socialism 101.

Trump just tried to fix this, but I think our big pharma slaves in Congress (prominently our own Senator Cassidy) blocked it.

I own a pharmacy that sells a lot of specialty (expensive) medicine. I have a lot of contracts with manufacturers. Let me explain the market. The reason our medicine is expensive is due to pharmacy benefits managers or PBMs. If your drug cost $1000 the PBMs gets about a 70% rebate or kickback from the manufacturer. The
PBM decides if your medicine is covered and for how much. The manufacturers have to pay them to get their drugs covered.
