I own a pharmacy that sells a lot of specialty (expensive) medicine. I have a lot of contracts with manufacturers. Let me explain the market. The reason our medicine is expensive is due to pharmacy benefits managers or PBMs. If your drug cost $1000 the PBMs gets about a 70% rebate or kickback from the manufacturer. The
PBM decides if your medicine is covered and for how much. The manufacturers have to pay them to get their drugs covered.

Let me explain the way our system works. The one making the money is definitely not the pharmacy. They are going out of business left and right. Big pharma makes a few dollars. The ones getting paid are the insurance companies. Of your $1600, the pharmacy benefit manager or pbm gets roughly a $1200 rebate from big pharma to put it on your formulary. If they don’t pay, your Eliquis comes back not covered. The PBMs are express scrips, Caremark, Aetna, optum and Humana. They only process the prescription but get paid the most. Then to make it even better, the do spread pricing, they send you a cob saying they paid the pharmacy $100 but only paid them $20. They scoop the rest. Don’t forget your insurance premium too. Oh wait, have you ever heard of a prior authorization? They get paid again to make you jump through hoops to get your prescription covered. That’s how the sausage is made. Sound unbelievable and illegal. Ever wonder why drugs out of Canada are 70-80% cheaper?
