I’m sure you know more than me, but the 10 or so married MD couples I know, both people work. One woman took an easier job after kid #4, but still works full time. I know more Stay at home dads that married a doc than I know female MDs that quit.

Good for you and her - Derm is where it's at. Probably the best at this current moment when you combine residency, lifestyle, and compensation.
Sometimes I wish I had gone that route, but then I remember I hate skin stuff.

I imagine it's a big step down to go from where you are, to a residency in the same specialty. Strange as it sounds.

I didn’t do an ER residency. I’ve just got a lot of ER experience moonlighting in small town ER’s and I just slowly moved up to bigger ERs

For smaller ERs you just need an unrestricted license to practice medicine and ACLS, ATLS, and PALS certification.
